We request that an insurance card is presented at each visit. All co-payments and past due balances are due at time of check-in unless previous arrangements have been made with the billing coordinator.
It is the responsibility of the parent to make sure the insurance is active the day prior to the appointment, otherwise the appointment will be cancelled unless you decide to pay our self-pay fee.
If the patient has a Medicaid insurance: it is the responsibility of the parent to make sure the office is assigned as PCP 24 hours prior to the appointment, otherwise, the appointment will be cancelled.
If you have a commercial/private insurance, it is your responsibility to know ahead of time if you have a deductible or copayments amount.
